How to lose weight on Mounjaro without losing muscle

Lose weight quickly on Mounjaro and some of it can be muscle. You keep more of it with three things: strength training two or three times a week, protein at every meal, and tracking your strength as well as the scales. Here is why Mounjaro muscle loss happens and what a plan that protects your muscle includes.

This guide is about training and eating while you are on a weight-loss medicine your prescriber has given you. It is not advice about the medicine or your dose. Speak to your prescriber or GP before you change your activity or diet, especially if you have a health condition.

Does Mounjaro cause muscle loss?

It can. In the main trial of tirzepatide, the medicine in Mounjaro, a body scan study found that about a quarter of the weight people lost was lean mass. Lean mass is everything that is not fat: water, organs and muscle among it.

That is not unique to the medicine. Any fast weight loss takes some lean tissue with it, because when you eat much less, your body uses what it has. The difference on Mounjaro is speed: the scales can drop faster than on a diet alone, so the question of what you are losing matters more.

Muscle is what keeps you strong on the stairs, steady on your feet and able to carry the shopping. It also makes the weight easier to keep off later. The aim is simple: let the fat go and keep the muscle.

GLP-1 and Wegovy muscle loss: the same rules apply

Wegovy, Ozempic and Mounjaro belong to the same family of medicines, often called GLP-1 medicines. They all reduce appetite, so you eat less and lose weight. Whichever one you have been prescribed, GLP-1 muscle loss comes down to the same two things: whether your muscles are being asked to work, and whether you are eating enough protein for them to hold on.

So everything on this page applies whether you are on Mounjaro, Wegovy or another GLP-1 medicine.

How to prevent muscle loss on Mounjaro

Three habits do most of the work. None of them needs a gym, and all of them fit around a small appetite.

  • Strength training two or three times a week. This is the signal that tells your body to keep its muscle while fat comes off.
  • Protein at every meal. Muscle needs it, and a small appetite makes it easy to fall short without noticing.
  • Tracking your strength, not just your weight. If your lifts hold steady while the scales drop, you are keeping muscle.

Strength training on Mounjaro: what a good plan includes

UK guidelines already ask adults to do muscle-strengthening work on at least two days a week. On a weight-loss medicine it matters more. A plan that protects muscle has these things in it:

  • Two or three full-body strength sessions a week, with rest days between them.
  • The big movements: a squat, a hinge, a push and a pull, set to your kit at home or in the gym.
  • Weights that get a little heavier over time, so your body has a reason to keep its muscle.
  • Shorter sessions for the days your energy and appetite are low, so you still train.
  • A daily walk on top.

The right exercises, sets and weights depend on your starting point, your kit and how you feel on the medicine, which is why we set them for you and change them every four weeks.

How to build muscle while on Mounjaro: protein first

Most UK pharmacies and dietitians writing about these medicines suggest 1.2 to 1.6 g of protein per kilo of body weight a day. When your appetite is small, that is hard to reach without a plan.

  • Eat the protein part of each meal first, before the rest of the plate.
  • Spread it across the day rather than saving it for one big meal.
  • Choose foods that go down easily when you are not hungry.

Your own target, and the meals and shopping list that hit it, are set from your weight, your appetite and what you like to eat. Your prescriber or a dietitian can advise you too.

How to lose weight on Mounjaro and know it is fat

The scales cannot tell fat from muscle. Track these as well, once every four weeks:

  • Your strength: the weights and reps on your main lifts.
  • Your waist, measured at the same point each time.
  • A front and side photo in the same light.
  • Your weight trend over the month, not a single weigh-in.

Signs you may be losing muscle: your lifts going down month after month, everyday things like stairs feeling harder, or the scales dropping while your waist barely moves. If you notice them, it is time to look at your training and your protein. If you feel faint, cannot keep food down or feel unwell, stop training and speak to your prescriber or GP.

How we help people on weight-loss medication

Zama Fitness runs a separate support plan for people losing weight with prescribed medication: a gentler strength and nutrition programme, reviewed by a registered dietitian, that works alongside your prescriber. You get your sessions, protein target and meals set for you, log them in the app, and your plan changes as your appetite and energy change. We do not prescribe medication.

Mounjaro muscle loss: your questions.

Does Mounjaro cause muscle loss?

When you lose weight quickly, some of it is lean tissue as well as fat. In the main trial of tirzepatide, the medicine in Mounjaro, about a quarter of the weight people lost was lean mass, which includes muscle. Strength training and enough protein are the two things you can do to keep more of it.

How do I prevent muscle loss on Mounjaro?

Train with weights two or three times a week, eat protein at every meal, and track your strength as well as the scales. Speak to your prescriber before you start a new exercise plan.

Does Wegovy cause muscle loss too?

The same applies to Wegovy and other GLP-1 medicines. They reduce your appetite, you eat less and lose weight quickly, and fast weight loss takes some lean tissue with it unless you give your body a reason to keep it.

How do I build muscle while on Mounjaro?

Strength training with weights that slowly get heavier, and protein at every meal. For most people on these medicines, keeping muscle while fat comes off is the goal, and a very good result.

What is Mounjaro muscle wasting?

People use it to mean losing a noticeable amount of muscle while on the medicine, so they feel weaker or look softer even as the scales drop. If you feel weak, faint or unwell, speak to your prescriber or GP.

Can I regain muscle after coming off Mounjaro?

Yes. Muscle responds to strength training at any age. Keep training, keep your protein up and expect your strength to climb steadily over the following months.
